Attach a form to an application

Ask application-specific questions by attaching a form to your application.

This feature is available on the Grow and Pro plans.

Add application questions

Before you begin, ensure the form you want to use has been published.

Open the application and select the Details tab.


Scroll to the New Applicants Form section. By default, this is set to No Form, meaning volunteers can apply using only their name and email address.


Select the form you would like applicants to complete from the dropdown and then click Save at the bottom of the page.


Once attached, volunteers will be prompted to answer the form questions as part of the application process. There is no separate form link to share. Instead, share the application URL with volunteers.

When applications are submitted, you'll be able to review both the application and form responses together in the Screening view.
